Community Liaisons LLC provides person-centered supports that promote independence, community connection, and meaningful outcomes. Services are delivered in alignment with the NJ Division of Developmental Disabilities (DDD) and Medicaid/HCBS standards.
Our team supports individuals and families across all NJ counties. We focus on safety, dignity, choice, and measurable progress—whether the goal is community participation, employment, skill building, or respite for caregivers.

Hands-on coaching and support to practice skills in real settings such as shopping, appointments, transportation use, and community navigation.
Structured activities that promote social connection, confidence, self-direction, and active participation in community life based on personal interests.
Assistance with understanding services, organizing supports, making informed choices, and navigating available resources in a practical and person-centered way.
Discovery, goal-setting, and employment planning to identify strengths, interests, work preferences, and realistic next steps toward meaningful employment.
Skill-building related to work readiness, including communication, task completion, following directions, workplace behavior, punctuality, and stamina.
Individualized job support to obtain and maintain employment, including coaching, check-ins, workplace problem-solving, and collaboration as appropriate.
Small-group employment support with structured supervision and coaching designed to build work habits, participation, and progress toward goals.
Support strategies that promote safety, improved coping, skill replacement, and positive outcomes, coordinated with the ISP and interdisciplinary team.
Training and guidance to strengthen informal supports such as family, friends, neighbors, and community relationships for long-term stability.
Short-term relief for caregivers while ensuring the individual continues to receive safe, supportive, and meaningful care during planned coverage periods.
