Community Liaisons LLC provides Home and Community-Based Services (HCBS) across all New Jersey counties. We partner with individuals, families, and Support Coordinators to build individualized plans that emphasize skill development, employment, community inclusion, safety, and natural supports.

Our operations align with DDD requirements, Medicaid HCBS rules, and agency policies covering training, background checks, incident management, HIPAA/PHI protections, and quality management. We actively collaborate with stakeholders to uphold individual rights and deliver dependable, respectful support.

Leadership

CEO: Rajab Musah — Licensed Pharmacist

Community Liaisons LLC is led by CEO Rajab Musah, a licensed pharmacist with extensive experience in community care, quality assurance, and client advocacy. This clinical foundation informs our approach to medication safety, care coordination, and practical, person-centered supports that respect each individual’s health needs, goals, and preferences.

Under this leadership, the agency emphasizes training, accountability, and continuous improvement to ensure reliable services and strong outcomes for the people we support.
