We offer Individual Supports, Community Inclusion, Respite, Supported Employment (Group & Individual), Pre-Vocational Training, and Career Planning statewide in New Jersey.
Are your services approved under CCP and SP?
Yes. We serve participants in both the Community Care Program (CCP) and the Supports Program (SP), following DDD standards and plan authorizations.
Which areas of New Jersey do you serve?
We provide DDD services statewide across all NJ counties, including Morris, Essex, Bergen, Middlesex, and beyond.
How can Support Coordinators make referrals?
Support Coordinators can submit referrals through our
online referral form; we acknowledge within 1–2 business days and coordinate next steps.
What information should I include in a referral?
Please include the NJSAMS ID (if available), services requested, approved units, staffing needs, risk considerations, and current plan goals.
What are your service start timelines?
Start dates depend on staffing, documentation, and participant preferences; we’ll create a mutually agreed timeline with the participant and coordinator.
Do you help with tenancy and housing navigation?
Yes. We assist with housing readiness, landlord outreach, and community resource navigation consistent with DDD and local program guidelines.
Is financial assistance for rent or deposits provided by Community Liaisons?
We do not directly pay rent or deposits; however, we help participants connect with eligible programs, benefits, and community resources.
Do you support transportation within services?
Transportation related to authorized services (e.g., Community Inclusion, employment support) can be included per the individual service plan and DDD rules.
Are you HIPAA compliant?
Yes. We follow HIPAA and NJ DDD privacy requirements for PHI, use secure transmission for referrals and documentation, and limit access to authorized staff only.
